  • Pickup address and destination
  • Whether the passenger uses a wheelchair or walker, or can walk with help
  • Whether a family member or caregiver will travel along
  • Insurance or Medicaid details, if you plan to use them

  • Rides are available 24 hours a day; the call center has set hours
  • Insurance and Medicaid accepted with a prior authorization letter from your insurer
  • A family member or caregiver may travel with you at no extra cost, subject to vehicle capacity
  • Please give us at least 24 hours' notice to cancel a scheduled ride
